There is growing recognition that consumers have enormous potential to influence their own health outcomes if they are actively involved in the decision making & provided with quality information & appropriate self-management skills

Community Health aims to achieve positive outcomes for consumers by working with the community to improve health & wellbeing. We do this by applying the Social Model of Health. Within this framework, improvements in health & wellbeing are achieved by addressing the social & environmental impacts on health in tandem with the physical & medical factors.

Basically, our role is to provide resources for you, the community, to tap into so that you can access the information & support you need to attain optimum health.

Health is a complete state of physical, mental & social wellbeing, not merely the absence of disease.

How does Dianella apply this? By including the community & local organisations/agencies in the planning of our services, through consultation & response to your needs. We have a wonderful, dedicated team of professional staff who are well supported by the Board & Management.

Hence we have a varied & comprehensive range of services which acknowledges the diversity of our community. Everything from GP’s and Podiatry to Turkish Women’s groups & Aboriginal Youth Arts projects.

Equal access for all, community participation in the development of programs that are identified by you & an underlying theme of health promotion which focus’s on prevention are all core commitments of our service provision.
