Dianella’s catchement area covers most of The City of Hume & surrounding suburbs. The populations living in these areas are diverse. Sustaining quality external relationships is a priority for Dianella, this is achieved through:

- Embedding accepted good practice
- Building community capacity.
Accepted good practice is built into all aspects of the organisation's strategic plan, policies and
procedures. Dianella recognises professional membership of its employees, subscribes to relevant resources; and participates in strategic peak bodies.
As a service provider and beyond, in specialist areas, Dianella is a leader, with credibility and social capital of positive influence. Community advisory groups with interests in health, self-help and volunteering, advise Dianella on specific programs at most centres. they regularly plan and review services and are surveyed annually for ways to strengthen their role and opportunities.
Dianella's strategic plan and other policies show its strong and active commitment to building and sustaining quality external relationships, partnerships and networks. There are clear protocols for working with external users and private providers. Funding is well monitored and relevant team managers negotiate project service outcomes.
As a significant partner in the PCP process, Dianella contributes actively to the research, planning, and development of service enhancement tools, processes and review. It sees itself as both a catalyst and participant in several shared programs and approaches to meet community needs most effectively.
Professional staff are encouraged to participate in external networks, committees and associations relevant to their work. These include Hume Child and Family Network; Hume Indigenous Advisory Committee, aged care, school networks, Neighbourhood Renewal, health promotion, the Senior Social Workers Network, Department of Human Services advisory committees, Primary Mental Health and HARP (hospital admissions risk program) governance. The CEO is involved in national and state community health and industry committees.
Dianella has strong links with the University of Melbourne, Latrobe University and the Royal Women's Hospital which contribute to research such as the after hours GP study, and student placements in many professions.
There is GP and HACC (Home And Community Care) accreditation. Programs are reviewed against best practice guidelines through the health promotion planning process. Staff are encouraged to participate in, and deliver, conference presentations, and training and study leave are well supported. External supervision is provided for all counselling and mental health staff, with senior practitioners participating in local professional networks.
Dianella is acknowledged as having special strengths in its substantial investment in training, and its planning and implementation processes aimed at building community and professional capacity.

Dianella demonstrates a commitment and understanding of primary health care principles. The service mix and multidisciplinary approach provide a strong foundation for a range of service interventions and activities.
Dianella has had a strong focus on improving and strengthening intake procedures in recent years, with intake planning groups and several designated positions. It has reviewed its waiting lists and come up with effective prioritising and referrals. Some programs co-ordinate assessments with other services. A multidisciplinary approach within Dianella itself has helped comprehensive care.
It commits considerable resources and expertise so consumers and community groups have access to a breadth of services, for example, community transport and a range of local sites.
Dianella has created a supportive environment for health promotion. the community/primary health care service:
- Promotes the health and wellbeing of its community of interest in a comprehensive way, by using a range of strategies appropriate to the health issues being addressed
- Contributes to the development and co-ordination of effective health promotion
- Has established reciprocal relationships with consumers, communities of interest and other stakeholders to make decisions, act on common goals and build community capacity.
Health promotion has wide-ranging strategies delivered at several sites, homes and schools; and reaches many people with screening, health education, community development, advocacy, capacity building and partnership strengthening.
