 Refugee Health Program


Program for newly arrived Refugees/Asylum Seekers. It provides an initial Health Assessment including Psycho/Social screening and provides links to primary health care services

Service Details:
The aim of the program is to provide information and access to Australian Health Care systems including the Special Medicare Item Refugee Health Check available through GPs to all refugee clients.
The program also provides referral to other Allied Health Services such as:

Optometry, Emergency Dental, Dental Waiting list, Physiotherapy, Audiology, Diabetes Nurse Educator, Dietitian, Podiatry and other services.

The program also provides a link to other Culturally specific social services.

Most clients are referred by their AMES Settlement Case Co-Ordinator within a week of arriving in Australia but the service is available to for clients up to 5 years after arrival.

Services available:

Provides Accessibility through the Arabic Access Worker and home visiting appointments.
Initial Nursing Physical and Psycho Social Health Assessment
Referral to GP for Refugee Health Assessment with provision of list of local Arabic speaking GPs and information no accessing Specialists.
Referral into relevant Allied Health Services listed above
Referral to other Social Support and Cultural specific services
Migrant Resource Centre, Foundation House for trauma Counselling, Victorian Arabic, Social Support, Cultural specific Ante – Post Natal programs
Assistance with Appointment booking & Interpreter provision
Translated Health Information on relevant agencies, services and specific health issues. E.g. Women’s Health, Pap Test, Cervical Vaccine, Breast Care.
Sunscreen – Skin Cancer in Australia, Immunization Timetable etc.

Eligibility: Available to all clients who have arrived in Australia in the past five years with a Refugee Visa or are seeking Asylum, and are settled in the City of Hume.

Referrals accepted from any source if the client is eligible.

Dianella Community Health has recently employed a Refugee Health Nurse. Part of this role will be to set up & link into refugee groups & communities. The nurse will work with the community to assist in providing better access to our services & to look at any specialised services that need to be made available. Settlement Grants Programme (SGP)- Youth Focus

Provision of Casework and Capacity Building for youth from the Humanitarian and Family stream entrants in Hume City.

Service Details:
The project focuses on entrants aged 12-25and their families, providing casework, information, advocacy and orientation to Australian culture and mainstream services.

Individuals and group work aims to provide ongoing support & networking for the target group to assist to integrate and become familiar with Australian society & culture.

Eligibility

Young people (12-25) years old and their family.
Refugees, migrants and newly arrived young people.
Referral: Contact the SGP - Youth Focus Worker at Craigieburn.
Referrals from school/teacher or TAFE, GP, family, employment agencies and other internal or external services. Self referral is encouraged.

Location: Craigieburn. Home visits can be organised.
Operating hours: Monday to Friday 9.30am - 5.30pm. Working hours may change during school holidays.
